The refit was completed in the spring of 1966.

Check out the Dover publication 'Picture History of the Cunard Line' by Bill Miller. It features a photo of the new Midships bar along with alterations made to other Cunarders in the mid-1960s.

The Midships Bar replaced the ballroom after the 1965-66 refit. The new bar was forward of the 1st class smoking room and aft of the 1st class main lounge (or Queen Elizabeth Lounge) on Promenade deck. The bar was decorated in green leather, yellow and orange with simple columns supporting the simple high white ceiling. The curved green leather bar was positioned aft w/dark rust-orange bar stools. The primary color combination used in the other 1st class public rooms were blues, dark red/rust, cream along with plenty of dark wood veneers w/nickel accents and fixtures.

There was a Writing Room(Portside adjacent to Salon), a Nursery(Starboard next to Lounge), a Salon with dance floor in Bewtween 1st class Lounge and 1st class Smoke Room and also smokestake.
Did Cunard removed a salon only?

The plans I have from the early 1960s still show the ballroom. The plans issued after the 1965-66 refit show the space as the Midships Bar. The basic floorplan along w/window placement was retained according to these two deck plans. Of course the furniture placement along with the new bar are shown in the newer deck plans. The 1965-66 plans show no dance floor in the Midships Bar as well as no room extensions (similar the the twin Garden Lounges off the smoking room) outward towards the sides of the ship. It is possible that she was altered again after 1966 but I do not have those deck plans.
The QM1's Observation Lounge and Cocktail Bar on Promenade Deck is shown in the First class deck plan(1/53) though deck plans of Tourist Class(9/64) is shown the Observation Lounge and Cocktail Bar(on Promenade Deck) and Garden Lounge(on Main Deck). The QE1's Observation Lounge is shown in the deck plans of 1st class(L.P. 39/181).
I read the twin level observation lounge which had for many years afforded a sophisticated view of great bow plunging into the waters of Atlantic had been converted in to a simple pub
